Dependent Life Insurance

Up to $300,000 in units of $10,000 for employee and / or spouse.

Weekly Indemnity

17-week or 26-week plan paying two-thirds gross earnings – up to a weekly max. of $2,000 (based on # of lives/employees)

Long-Term Disability

OPTIONAL BENEFIT

Benefit: 5-year max or to age 65, benefit period paying 60% or two-thirds gross earnings to a monthly max of $8,000 (based on # of lives/employees)

Benefit ceases at age 65 or earlier retirement

Full Semi-Private - no deductible - no daily max.

120 continuous days per trip / $5 million lifetime.
